Advance care planning in the palliative phase for people with intellectual disabilities: development, evaluation, and implementation of an ACP-program

Advance Care Planning (ACP) in the palliative phase means that professionals communicate with clients and relatives in advance about wishes and preferences for future care. ACP is not self-evident for people with intellectual disabilities (ID).
How can ACP in palliative care be implemented?

The project consists of multiple steps
(1) a systematic review of the literature;
(2) analysis of medical records and interviews;
(3) development of an ACP-program;
(4) implementation and evaluation of the ACP-program.

The project results in two scientific international publications and a free accessible ACP-program.
