Oncology after care in primary care

After care and follow-up for cancer patients is a fast growing theme in primary care. The number of cancer survivors will increase considerably. Besides, follow-up care for cancer is being transferred from secondary to primary care. An overview of the current situation of after care for cancer patients in primary care, including bottlenecks and a research agenda will support the discussion about the future of after care and follow up for cancer patients
This synthesis of available knowledge will answer the following questions:
• what is the current situation of after care for cancer patients delivered by primary care, including general practitioners, district nurse and paramedics.
• How do primary and secondary care join forces in follow-up?
• What are the experiences of cancer patients regarding after care.

Abovementioned questions will be answered based on extant literature, several interviews and online focus-group discussions with stakeholders. During an invitational conference results will be presented and recommendations and conditions for after care and follow-up for cancer patients in primary care will be determined together with the participants.
