|Project description||The project aims to realize appropriate care in hospital settings that meets the needs and care needs of patients in the final stage of life.
We realize this by
(1) Providing insight into how patients, family members, doctors and nurses can contribute to appropriate care through joint decision-making about whether or not to continue treatment in the final phase of life;
(2) Developing and implementing a practical tool, including educational material, about how to support joint decision-making about appropriate care, in which nurses have a substantial role.
A so-called co-design panel, consisting of representatives of patients, relatives, nurses, doctors and trainers, are involved in all phases of the project. The (further) development of the tool and the associated training takes place in close cooperation with the co-design panel.
