Agreement in doctor-diagnosed and self-reported morbidity in the Dutch National Study of General Practice.

Background: To obtain information about the prevalence of chronic conditions, data can be acquired both from medical records and from health of either method. Methods: The participating general practitioners recorded morbidity data,
using the International Classification of Primary Care (ICPC) in all consultations and during one year. DNSGP-2 includes a health interview survey carried out in 2001 among a 4% random sample of the practice population of the 195 participating general practitioners. Survey-data were obtained from 12.699 respondents (response rate 65%). Comparisons on the presence of chronic conditions were made on individual level of patients (25 and over). Explanatory variables in multivariate logistic regression analyses included different patient- and doctor characteristics. Results: About 65% of the interviewed population reported at least one chronic condition. Most frequent self-reported chronic conditions were migraine/headache (18%), rheumatic complaints (15%), high blood pressure (14%), neck/shoulder complaints (14%) and severe back complaints (14%). The GP was consulted by 80% of the practice population, and most frequently for hypertension (10%), severe back complaints (9%), neck/shoulder complaints (8%), eczema (7%). Conclusions: Results show that for some chronic conditions, prevalence of GPdiagnosed and of self-reported chronic conditions have considerable overlap, others therefore differ extremely. Preliminary analyses show that both patient- and doctor characteristics explain variances in agreement.(aut. ref.)
