Policy concerning person-centred care for people with multimorbidity

Background

As part of the EU funded ICARE4EU project, Nivel developed a policy brief on patient-centred multimorbidity care together with consortium partners (including the TU Berlin) in 2016. This policy brief was published by the European Observatory on Health Systems and Policies. The Observatory has requested an update of this policy brief, focusing on effective strategies for implementing person-centred multimorbidity care.

Research question

How can person-centred care for multimorbid populations be implemented at various levels in European countries, to improve the quality and outcomes of care for these populations?

Method

To update the policy brief and provide more information on effective strategies to implement person-centred care, a systematic review is conducted, good practices (developed in EU funded projects and joint actions) are reviewed and relevant documents on person-centred care of other international organisations (e.g. WHO, OECD) are studied.

Result

An updated policy brief, including policy recommendations and pragmatic guidance on how policymakers and care providers at a local, regional or national level could develop and implement person-centred multimorbidity care in close collaboration with patients and carers.
